Open LPN Positions:

• Full Time Afternoon/Evening shifts
• Part Time Afternoon/Evening and Overnight shifts

CCLPN (Care Coordinator Licensed Practical Nurse): Provides services to individuals in their own homes and communities, who need assistance with their activities of daily living as a result of sickness, old age, disability and/or other inflictions.

CCLPNs are direct care providers who are knowledgeable on individual patient needs who then coordinate services through effective partnerships with patients, their caregivers/families, community resources, and their physician or other providers.

Experience in medical oversight and monitoring regarding to UTI's, Diabetes, Parkinson's, COPD and other medical conditions for the elderly is required. Care, or Hospice experience is a plus.
